How We Care

Our Model of Care

We trace our success as a company to the simple and enduring philosophy that aging is a gift. The wealth of knowledge and experience that comes with age is a gift to the individual & the family.

We work to provide an environment that supports the whole person—mind and heart, as well as body.

You’ll work with a care coordinator at your local office to set a care schedule.

“Caring for seniors is a labor of love and requires a special person”

Everything we do is driven by our universal mission: to enhance the lives of ageing adults and their families. The solution for families that need an extra level of care. The place we call home is the place that feels most comfortable and secure.

A sense of home can become especially important as we age. Our in-home senior care services help ease the challenges of ageing while encouraging seniors to enjoy the comforts of home.

“Caring for seniors is a labor of love and requires a special person”

Through more than 20 years of providing innovative solutions that improve health and quality of life for those in need of in-home health services, PathWell - Senior Care Center has established itself as one of the most experienced homecare companies in the industry.

Our approach to quality care focuses on a commitment to providing a level of expertise, training, patient service, and monitoring that is difficult to match in our industry.

“Caring for seniors is a labor of love and requires a special person”

Elderly care, or simply eldercare (also known as aged care), is the fulfillment of the special needs and requirements that are unique to senior citizens. This broad term encompasses such services as assisted living, adult day care, long-term care, nursing homes, hospice care, and home care.

Because of the wide variety of elderly care found nationally, as well as differentiating regional perspectives on elderly citizens, it cannot be limited to any single practice.

What People Say

To ANHF Home Care Team,

After a mild stroke I had difficulty with my mobility. Care workers from ANHF have helped me a lot with medical appointments, attending seniors’ social activities, shopping, etc. She never complains despite having to push my wheelchair everywhere I go. She waits until I am inside the house, before leaving. Without her support, I’ll be homebound. In short, her service is excellent.

Nan spent her last years at Bernard Chan Nursing Home, where she was later joined by Kevin (Grandpa), and where she passed away surrounded by family. She was very well looked after there and we are grateful for the care that they gave her.

Zhong Min (room 25A) had a fulfilling family time during his five-year stay in Bernard Chan Nursing Home. The Directors of Nursing led their professional teams in providing quality care for every resident, responded to all our concerns with solutions and pleasant smiles. We felt so grateful for such tender loving care. Here in BCNH is indeed a paradise on earth, a place where different daily programs inject happiness and joy in the lives of residents!

While Ah Bo always works in the lounge area with a happy face despite the busy tasks, nurses carefully assist residents in their medication needs, treating them as if they were their own family members. New programs are on display on the notice board, residents recall fond memories through various festive programs conducted by staff.

I have to thank all the DONs for getting close to our hearts, sharing our happiness and sorrows, lending a helping hand to our worries. Seeing the excellent models from leaders, the staff follow their examples and treat every resident with equality, all contributing to make BCNH a home of vivacity and love.

Bygone nights and distant music are eternal, yet people who leave in the course of time. There is no way to go against the law of nature… May the flowers bloom forever, the moon shines till eternity, mankind has longevity!
